What is PCMH?
The Patient-Centered Medical Home (PCMH) is a care delivery model where a patients treatment is coordinated through their primary care provider to ensure they receive the necessary care when and where they need it, in a manner they can understand.
The objective is to have a setting that facilitates partnerships between individual patients, and their personal providers, and when appropriate, the patient’s family. The primary care medical home demonstrates a commitment to quality and quality improvement by ongoing engagement in activities such as using evidence-based medicine and clinical decision-support tools to guide shared decision making with patients and families, engaging in performance measurement and improvement, measuring and responding to patient experiences and patient satisfaction.
PCMH is a model of care that puts patients first while helping to build better relationships between patients and their clinical care teams.
